Integrated Behavioral Health Services
The primary goal of integrated behavioral healthcare is to improve communication between behavioral health and primary care providers and thereby improve care coordination and most importantly patient health outcomes. Research has shown that on average 30-50% of patient referrals from their primary care provider to an outpatient behavioral health clinic do not make their first appointment. Uniquely, the Wellcova Health | Lowry Pediatrics partnership offers a co-location with your primary care provider and behavioral health clinician, allowing familiarity, and better continuity of care, all of which improves overall patient health outcomes.
Multidisciplinary care is an integrated team approach to healthcare. In multidisciplinary care, the development of patient-centered treatment plans and delivery of care becomes a shared responsibility. The evaluation of treatment options and treatment planning is a collaborative process that involves patients, their families, and all health care providers involved in the patients care.
Medication Management
Medication Management at Wellcova involves prescribing and assessing all the medication a patient takes (prescription, over-the-counter, and supplements) to determine any potential interactions while monitoring for effectiveness and side effects. Key components include:
- Medication Review: Regularly assessing all current medications to ensure they are necessary and effective.
- Patient Education: Ensuring patients understand their medications, including how and when to take them, potential side effects, and the importance of adherence.
- Monitoring and Adjustment: Keeping track of how well the medication is working and making adjustments as needed.
- Coordination with Providers: Collaborating with doctors, pharmacists, and caregivers to optimize a patient’s medication regimen.
This process is vital for managing chronic conditions, preventing medication errors, and improving overall health outcomes.
